  • A face mask, that covers both mouth and nose, must be worn at all times while in the clinic.
  • If you have tested positive for COVID at anytime we will need to see a copy of the recent negative result before you will be able to be seen.
  • If you are travelling from any other state, you need to bring proof of a negative COVID-19 test that has been performed within 3-5 days of your appointment.
